I really like the Singapore Math Workbooks and blog about it at http://pragmaticmom.com under math workbooks. I also like Daily Word Problems and Life of Fred: Math as Simple as it needs to be.
My kids do math supplementation during the summer so they can remember what they've learned plus a little during the school year.
I also have a list of picture books that teach math concepts which I found to be a fun way to integrate math at bedtime.
